According to data compiled and analyzed by Madison Dapcevich, an Associate Editor at Outside, in collaboration with leading river safety organizations, the number of fatal accidents involving paddleboards has quadrupled over the last five years. The primary culprit identified by experts is not a lack of flotation, but rather the entrapment caused by ankle leashes in river environments. Data published by American Whitewater, a preeminent nonprofit dedicated to river conservation and safety, reveals a chilling trend: between 2015 and 2020, there were only three recorded fatal SUP accidents involving ankle leashes. However, between 2020 and July 2026, that number surged to at least 12. This does not include the additional fatalities involving river surfboards, where at least two individuals died under similar circumstances while wearing ankle leashes in the same timeframe.

The geographical epicenter of this crisis has recently been the Pacific Northwest. In Oregon alone, there have been five fatal ankle-leash SUP accidents since 2020. These statistics have prompted American Whitewater and other safety advocates to issue urgent warnings to the public: ankle leashes have no place on moving water, regardless of how "lazy" or "flat" a river might appear to the untrained eye.

The Mechanical Failure of the Ankle Leash

To understand why a safety device becomes a hazard, one must look at the mechanics of river flow versus ocean swell. Ankle leashes were originally engineered for the surf. In an ocean environment, water moves in an oscillatory motion; waves come in and pull back, and a fallen paddler needs their board close by to avoid a long swim through breaking waves. If a leash gets snagged on a reef, the surge of the water often provides a momentary slack that allows for release.

Rivers are a different beast entirely. A river is a unidirectional force of nature—a "conveyor belt" of energy that never stops. When a paddler falls off their board in a river, the board and the human are swept downstream. If the leash or the board becomes snagged on a "strainer" (such as a fallen tree), a rock, or a bridge piling, the current immediately pulls the paddler’s body away from the snag point. Because the leash is attached to the ankle, the force of the water pins the paddler’s leg downstream, while the board remains stuck upstream or vice versa.

Evan Stafford, a spokesperson for American Whitewater, emphasizes the physical impossibility of self-rescue in these scenarios. "Ankle leashes should never be worn in moving water, even on rivers that appear to be flat and very slow-moving," Stafford explained. "Any type of leash can become entangled in rocks or wood debris. When ankle leashes get hung up, reaching back upstream to the ankle cuff to release it against the force of the current is often impossible."

The Physics of Drowning: 22 Pounds of Pressure

The danger is not merely theoretical; it is rooted in the overwhelming physics of moving water. Many beginners underestimate the power of a slow-moving river. Barry Hughes, a renowned whitewater paddleboard coach and veteran water safety campaigner, describes the sensation as a losing battle against an invisible giant.

"To give you an idea of what it’s like in a flow of just three miles per hour: imagine trying to do a sit-up with 22 pounds of water pressure hitting you in the face and chest whilst trying to breathe," Hughes noted. In this scenario, every time a paddler attempts to reach down toward their ankle to undo the Velcro strap, the current pushes their torso further under the surface. In a river, the "suction" created by the water moving around a human body can keep them pinned underwater indefinitely. As Hughes points out, many rivers and tidal estuaries flow much faster than three miles per hour, exponentially increasing the force exerted on the trapped individual.

The Explosive Growth of the SUP Market

The current safety crisis is a byproduct of the sport’s own success. While stand-up paddleboarding has roots stretching back centuries to Hawaiian "Hoe He’e nalu" traditions, it didn’t enter the American mainstream until the 1940s on Waikiki Beach. For decades, it remained a niche coastal activity. However, the last decade has seen a revolution in the sport, driven largely by the development of high-quality Inflatable Stand-Up Paddleboards (iSUPs). These boards are portable, affordable, and have opened the doors for millions of inland residents to take to their local waterways.

By 2026, an estimated 4 million people in the United States alone participate in SUP activities. A 2025 report from the American Canoe Association (ACA) highlighted that stand-up paddling maintained the highest growth rate among all outdoor activities, with participation increasing by an average of 26 percent between 2022 and 2024.

This rapid expansion has created a "knowledge gap." New paddlers often purchase "all-in-one" kits from big-box retailers or online marketplaces. These kits almost universally include an ankle leash because it is the cheapest and most common tethering device. Manufacturers, often based in regions with more surf than river culture, do not provide adequate warnings about the specific dangers of using these leashes in moving water. Consequently, a beginner may take their new board to a local river, thinking they are being safe by "buckling up," only to inadvertently strap themselves into a potential death trap.

The Solution: Quick-Release Waist Belts

Safety experts are not suggesting that paddlers abandon leashes entirely—rather, they are calling for an industry-wide shift toward quick-release (QR) waist belts. A quick-release system involves a belt worn around the waist with a mechanical cam buckle. The leash is attached to the belt rather than the ankle.

The advantage of this design is two-fold. First, the release mechanism is located at the waist—the center of the body—where it is easily accessible by either hand, even if the paddler is being buffeted by current. Second, the release is a "single-pull" system; one tug on a toggle opens the buckle, instantly separating the paddler from the leash and the board.

"These have automatic release systems that give you a better chance to release yourself from the board if it gets stuck," Hughes said. He argues that waist leashes should be the industry standard for all SUP packages sold to consumers. "As a rule, someone buying a SUP package is a beginner, so equipping them with a waist leash is, in my opinion, the safer option."

However, even with a quick-release belt, proper usage is critical. Experts recommend that the belt be adjusted so that no more than two inches of webbing extend through the buckle. This prevents the excess "tail" of the belt from getting tangled in the mechanism during an emergency. Furthermore, paddlers must practice the release motion until it becomes muscle memory. In a high-stress entrapment scenario, there is no time to fumble for a toggle.

A Comprehensive Approach to River Safety

While the leash is a primary focus, experts like Stafford and Hughes stress that gear is only one part of the safety equation. The "leash-first" mentality can sometimes lead to a false sense of security.

  1. The Choice Not to Wear a Leash: In many high-volume or high-velocity whitewater environments, the safest choice is often to wear no leash at all. While losing a board is an expensive inconvenience, it is preferable to being tethered to a submerged object in a Class III rapid.
  2. Personal Flotation Devices (PFDs): A life jacket is non-negotiable. Many SUP fatalities occur because the paddler, thinking the board is their primary flotation, neglects to wear a properly rated and fitted PFD. If a paddler is separated from their board, the PFD is their only lifeline.
  3. River Literacy: Understanding the "language" of the river is essential. Paddlers should be able to identify "eddies" (safe zones), "strainers" (dangerous obstructions), and "low-head dams" (often called "drowning machines").
  4. Professional Instruction: As the sport moves into more technical environments, the need for formal training grows. Many local outfitters now offer "River SUP" clinics that teach specifically how to navigate currents, perform ferry angles, and execute self-rescue maneuvers.

The rise in SUP-related deaths is a sobering reminder that as technology makes the outdoors more accessible, the responsibility for education must keep pace. The transition from ocean waves to downstream currents requires more than just a change in scenery; it requires a complete overhaul of the paddler’s safety toolkit. By ditching the ankle leash in favor of quick-release systems and better river education, the paddling community can ensure that the sport’s growth isn’t measured in lives lost, but in miles safely traveled. As Barry Hughes aptly concludes: "You need to take responsibility for your own safety. Understand the water you are paddling on, and use the right equipment for the environment."
